Password Vault is not filling out the username and
password after navigation to the website?
Verify that Internet Explorer is NOT in Protected Mode, this will prevent
automation of the login.

The import from CSV file expects the following headings to exist in your file,
add these as column headers if necessary:
category (this will become a folder in the tree)
description (this is the name of the website or account)
username (optional)
password (optional)
url (web address is optional)
